Foster Care Immediate Enrollment Form

This form is designed to ensure that children in the custody of a Nevada county child welfare agency are promptly and appropriately enrolled in school when entering foster care or undergoing a placement change. The form collects essential student and agency information, certifies the child’s eligibility for immediate enrollment regardless of missing records, and facilitates coordination between child welfare and educational agencies. Enrollment must occur as soon as possible—ideally by the next school day—and cannot be denied due to incomplete documentation. Schools must expedite educational record transfers and hold an Educational Services Meeting within 30 days to address the child’s academic, social, and transportation needs.
